29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: Dexter Dam- (a) The Hartzell ventilation fan blades were exposed through 5/8" openings on the guard, exposing workers to serious injuries. The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 212(a)(5), which was contained in OSHA Inspection Number 96892, Citation Number 1, Item Number 6, and was affirmed as a final order on 09 /16/2011 with respect to a work place located at, Umatilla, OR. Note: Abatement certification and supporting documentation are required for these items.
