29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  Hills Creek Dam-   (a) The work platform used for blasting was located 10' 2" from the floor and equipment below and was not equipped with standard railings.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Hills Creek Dam-  (a) The Baldor pedestal grinder's tongue guard was measured at 3/4" exposing operators to serious injuries.    The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 215(b)(9), which was contained in OSHA Inspection Number 941527, Citation Number 2, Item Number 3 , and was affirmed as a final order on 12 /11/2013 with respect to a work place located at, Rufus, OR.  Note: Abatement certification and supporting documentation are required for these items.

The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 212(b), which was contained in OSHA Inspection Number 741082, Citation Number 1, Item Number 1 , and was affirmed as a final order on 04 /26/2013 with respect to a work place located at, The Dalles, OR.  Note: Abatement certification and supporting documentation are required for these items.
