29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. (a) The employer did not ensure that Loeser polisher process specialists were properly trained to determine if the specialist followed the LOTO procedure to isolated all power sources. This violation occurred on or about 03/11/2015, when an process specialist was servicing the unit and did not LOTO all the power sources to the Loeser polisher.
