Defective rigging equipment shall be removed from service:    On or about April 1, 2015, located on the West Side of a H-Frame Self Supported Cell Tower employee(s) involved in an antenna pipe rigging and lowering operation utilized 2" tape and loaded hoist line rope rigging material method which was defective and not removed from service in that the tape material utilized to secure the load line to the antenna pipe was insufficiently adequate exposing employees working below to the hazard associated with elevated falling antenna pipe from the failed defective rigging material.
