29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: On or about April 1, 2015, located on the West Side of a H-Frame Self Supported Cell Tower employees were performing rigging operations that had not been trained on the applicable procedures or regulation to minimize those hazards associated with rigging.

29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service: On or about April 1, 2015, located on the West Side of a H-Frame Self Supported Cell Tower employee(s) involved in an antenna pipe rigging and lowering operation utilized 2" tape and loaded hoist line rope rigging material method which was defective and not removed from service in that the tape material utilized to secure the load line to the antenna pipe was insufficiently adequate exposing employees working below to the hazard associated with elevated falling antenna pipe from the failed defective rigging material.
