29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     At or about April 3, 2015 and times prior thereto, at the worksite located at or near Lot 30, 321 Miramar Boulevard, Little Rock, Arkansas 72223, employees working on a pitched roof engaged in roof shingling activities were not wearing fall protection and were not protected from falling. Employees were exposed to an approximately eight foot fall from the roof.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At or about April 3, 2015 and times prior thereto, at the worksite located at or near Lot 30, 321 Miramar Boulevard, Little Rock, Arkansas 72223, ladder side rails did not extend at least 3 feet above the roof. This condition exposed employees to fall from height hazards. This condition was observed in the following instances:    Instance A: An aluminum extension ladder near the front entrance of the house being built.    Instance B: An aluminum extension ladder near the garage of the house being built.    Instance C: An orange fiberglass extension ladder near the garage of the house being built.
