29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.    The employer did not performed an engineering survey to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to allowing employees to start demolition operations at 2501, 2525 and 2601 N. Mesa in El Paso TX. 79902.  This condition exposed employees to struck-by or crushed by hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    Employer allowed employees the use of a 24 foot extension ladder that was not extended at least three feet above the upper landing surface of the building located at 2601 N. Mesa, El Paso TX. 79902.  The extension ladder could not reach as to leave the 3 feet above the landing surface when fully extended.  This condition exposed employees to fall hazards.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103)(a).     The employer did not establish a written respiratory protection program with provisions included under 29CFR 1910.134 (C)(i)-(ix), and worksite specific procedures where the potential for silica overexposure existed during the demolition work that was performed at 2501, 2525 and 2601 N. Mesa, El Paso TX. 79902, nor implemented them when the use of respirator was part of the demolition plan developed by JMR Demolition, Ltd. submitted as part of the personal protection equipment for employees.  This condition exposed employees to health hazards.
