29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) On or about 04/07/2015 at the apartment complex, building #600, employees were exposed to a 30 ft. fall hazard, in that, fall protection was not used. The lanyards/lifeline were not connected to the harnesses. Employees were installing shingles on a 5 in 12 pitched roof.
