OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to crushing and struck-by hazards:    a)Main Bay Areas: 2 Rotary in-ground automotive vehicle lifts (385133) were missing mechanical locking pawls.  Employees were permitted to work under automotive lifts which were not mechanically secured from lowering on to an employee.  Observed on or about 2/25/15.    b)Main Bay Areas: 2 Rotary in-ground automotive vehicle lifts (385133) locking pawls did not engage.  Employees were permitted to work under automotive lifts which were not mechanically secured from lowering on to an employee.  Observed on or about 2/25/15.    c)Main Bay Areas: 2 Rotary above ground automotive vehicle lifts (JG1129) were missing mechanical locking wires for mechanical locking devices.  Employees were permitted to work under automotive lifts which were not mechanically secured from lowering on to an employee.  Observed on or about 2/25/15.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a)Rear corner of the building (used oil storage area): Employer did not have an operable eyewash that would provide fifteen minutes of continuous flow where employees worked with corrosives such as but not limited to car batteries, motor oils, and other injurious corrosive materials used for various processes during vehicle maintenance. A 120V outlet in a completed installation was used to power various machinery including but not limited to a tire mounting machine and a wheel balancing machine without a faceplate installed.
