Tiede-Zoeller Tile Corporation, 101 East 6th Street, Erie, Pa, 16501,1st floor of Erie Insurance Heritage Center, on or about March 31, 2015, and times prior thereto: Employees were working with and around corrosive chemicals including, but not limited to, Key Resin Company brand #108 Epoxy Terrazzo Part B Hardener. Employees did not have close access to a suitable facility for quick drenching or flushing of the eyes and/or body. Additionally, employees were unaware of the location of quick drenching and flushing facilities.

The employer did not make arrangements for prompt medical attention for the job site in that, a suitable facility for quick drenching or flushing of the eyes and body was not available for immediate emergency use.
