29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:     On April 7, 2015 employees installing decorative stone were not protected from a 21 foot fall while working from a metal frame scaffold and the employer did not ensure that employees were protected from falling to a lower level. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    On or about April 7, 2015, employees using Type N mortar mix while laying chimney brick were exposed to the hazards associated with silica and hexavalent chromium and the employer did not provide hazardous chemical training, labeling, and safety data sheets to employees working with these contaminants.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
