29 CFR 1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:    a) Assembly Department:  Employees hands and fingers were exposed to the unguarded moving clamps and levers of the Dodds Drawer Box Press (serial number C94010-258 and model #C36J) while assembling a standard-sized drawer, exposing employees to an amputation injury, on or about April 7, 2015.     b) Assembly Department:  The moving clamps and levers of the two large Drawer Box Presses: Cieffe 2000 Press S/N: 16969 and Divisione Montaggio Press CF2000 Model #242/87 were not guarded as observed, on or about April 7, 2015.                               ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) At the facility: The employer did not establish a Lockout/Tagout Program consisting of energy control procedures, employee training and periodic inspections. Employees were exposed to the release of hazardous energy while performing service and maintenance to machines such as the Dodds Drawer Box Press, on or about April 7, 2015.                                ABATEMENT CERTIFICATION REQUIRED FOR THIS VIOLATION.
