29 CFR 1926.651(k)(1):  Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a  competent person for evidence of a situation that could result in a possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazards conditions.  An inspection shall be conducted by the competent person prior to the start of work and as needed through the shift.    AVD: The competent person failed to conduct an adequate inspection of the excavation and incorrectly relied on the valve vault to protect employees fro ma possible cave-in.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
