On or about 4/8/2015, on the 4/12 pitched roof where shingles and roofing materials were being removed fall protection was not provided, exposing employees to an 8-1/2 foot fall hazard.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: a. On or about 4/8/2015, employees used a portable extension ladder to gain access to the 4/12 pitched roof without the ladder being secured from displacement, exposing employees to an 8-1/2 foot fall hazard.
