29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about 4/8/2015, on the 4/12 pitched roof where shingles and roofing materials were being removed fall protection was not provided, exposing employees to an 8-1/2 foot fall hazard.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a. On or about 4/8/2015, employees used a portable extension ladder to gain access to the 4/12 pitched roof without the ladder being secured from displacement, exposing employees to an 8-1/2 foot fall hazard.
