29 CFR 1926.405(d): Panelboards mounted in cabinets, cutout boxes or enclosures designed for the purpose were not dead front: (a) Near the Boiler Room Door - An energized 600 volt circuit breaker box was not properly covered and had exposed live parts, potentially exposing employees working in the area to a serious hazard, observed on or about 4-8-15. (b) In the Boiler Room - An energized 600 volt circuit breaker box had a cover that was too short for the box and had exposed live parts, potentially exposing employees working in the area to a serious hazard, observed on or about 4-18-15. Abatement Certification Required.
