29 CFR 1910.178(a)(5):  Powered industrial truck(s) equipped with front-end attachments other than factory installed attachments require the user to request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  A.  On or about April 9, 2015, Receiving Area:   1.  CAT GP20K with standard 42 inch forks did not have the 96-inch fork attachments identified on the data plate.  2.  CAT 50 GP25 with 45 inch forks did not have the 96-inch fork attachments identified on the data plate.

On or about April 9, 2015, in the following locations, overcurrent devices were not readily accessible:  1.  Cutoff Room:  120/208V electrical panel powering shop equipment was not readily accessible, in that a green plastic container, cardboard waste barrel and cardboard box were located in front of it.  2. Tool & Die Area:  120/208V electrical Panel 9 was not readily accessible in that a blue recycle bin was located in front of it.  3. Tool & Die Area: 120/208V electrical Panel 7 was not readily accessible in that a welding cart had been placed in front of it.   4.  Outside Polishing Room:  An unlabeled 120/208V electrical panel  was not readily accessible in that cardboard boxes of parts were stored in front of the panel.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  A.  On or about April 9, 2015, Receiving Area:  Insulation for the cord powering the lift on the Niagara squaring shear had been pulled from its fitting, exposing employees to the energized wires and electric shock hazards.
