On or about April 8, 2015, employees operating powered industrial trucks such as but not limited to, container handlers, front loaders, and yard hustler's were not trained, exposing employees to safety hazard.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: a. On or about April 8, 2015, in the yard area where vehicular traffic and forklift trucks operate, the passageways contained accumulated amounts of debris, trash and other objects. The working areas were not cleaned routinely nor maintained in a orderly and sanitary condition.
