29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  a. On or about April 8, 2015, employees operating powered industrial trucks such as but not limited to, container handlers, front loaders, and yard hustler's were not trained, exposing employees to safety hazard.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:  a. On or about April 8, 2015, in the yard area where vehicular traffic and forklift trucks operate, the passageways contained accumulated amounts of debris, trash and other objects. The working areas were not cleaned routinely nor maintained in a orderly and sanitary condition.
