29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:     a.  At the job site on the first floor and mezzanine area, employees were not protected with hard hats while performing demolition activities, thereby exposing employees to a struck-by hazard.

29 CFR 1926.102(a)(1):  Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:     a.  At the job site on the first floor and mezzanine area, employees were not protected with safety glasses at a minimum while performing demolition activities such as using a reciprocating saw, thereby exposing employees to a struck-by hazard.
