29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed; a) Front Center Roof of Structure - The ladder used by the workers to gain access to the rooftop deck work area, 35' above ground, was approximately 12" short of landing surface, on or about 4/07/2015.
