29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: Worksite located at 624 SW 6th St. Oklahoma City, OK 73160. On or about April 7, 2015, employees of the subcontractor were observed working from the ground without the use of protective helmets while other employees of the subcontractor were working from the roof level. The subcontractor's employees were exposed to struck-by hazards of falling objects from the roof.

On or about April 7, 2015, employees of the subcontractor were observed installing roofing shingles without the use of fall protection. The subcontractors employees were exposed to a fall hazard or more than 6 feet.
