On or about April 7, 2015, employees were observed working from the ground without the use of protective helmets while other employees worked from the roof level. The employees were exposed to struck-by hazards.

On or about April 7, 2015, the portable ladder used did not extend at least 3 feet above the landing surface. The employees accessing the roof were exposed to a fall hazard.
