29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. Location: 331 Madison Ave., New York, NY. a) Employer did not provide a cover for an electrical panel to protect employees against accidental contact. The panel was installed at height of approximately 5 ft from the floor at a hallway that employees walked by daily. Employees were exposed to an electrical shock hazard; on or about 4/7/15.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. Location: 331 Madison Ave., New York, NY. a) Employer did not protect employees from falling while working at an unprotected wall opening at the site. After the non-bearing wall was demolished, the outside bottom edge measured approximately 12 ft and the inside bottom edge measured approximately 27 inches; on or about 4/7/15.
