29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  a)  Spice packaging areas:  On or about April 7, 2015 and at times prior, the employer exposed employees to falls of up to 6 foot 2 inches while working from the top of the spice packaging machine identified as PK-06 without rails or equivalent fall protection.  b)  Flour packaging areas:  On or about April 7, 2015 and at times prior, the employer exposed employees to falls of up to 6 foot 3/8 inch while working from the top of the Flour packaging machine identified as PK-01 without rails or equivalent fall protection.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location:  a)  Bake Lab, left side behind sink:  On or about April 7, 2015 and at times prior, the employer exposed employees to electrical shock hazards in that all receptacles located behind the sink in the bake lab were not protected by ground fault circuit interrupter (GFCI).

29 CFR 1910.134(c)(2)(i): The employer did not provide respirator users with information contained in Appendix D to this section("Information for employees using respirators when not required under this standard") when voluntary respirator use was permissible:  (a)  Facility wide:    On or about April 7, 2015, and at times prior, employees were allowed to use an N95 style respirator, specifically the ProCare CN95 mask, without with out providing employees with the information contained in Appendix D.
