29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 7, 2015, on the east side of the commercial building, employees engaged in demolition activities on a roof pitch of 4:12 without fall protection were exposed to a fall hazard of approximately 18 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about April 7, 2015, on the east side of the commercial building, employees using a 10 foot A-frame ladder that did not extend 3 feet over the edge of the roof were exposed to fall hazards of greater than 6 feet.
