29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s) (a) 230 Obie Road, Schaefferstown Pennsylvania, Truck #8 - Employees were not warned by the posting of danger signs that the fuel tank was not an appropriate location for riding on the recycling truck, on or about March 30, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) Hammock Way, Lancaster Pennsylvania - An employee suffered a hospitalization on March 30, 2015 as a result of an injury from a fall off a recycling truck. The employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on April 1, 2015.) No abatement certification or documentation required.
