29 CFR 1910.36(g)(2): The exit access was not at least 28 inches (71.1 cm) wide at all points:      Employees were exposed to hazards related to exiting the facility in the event of an emergency because the following locations were not at least 28 inches wide at all points:      a. The aisle way next to the blow mold machine on the north side was restricted to 25 inches      b. On the north side of the blow mold machine towards the back corner there were boxes were stacked restricting the path to 24 inches.      c. The south side aisle way was restricted to 12 inches because a plastic tub was positioned next to the blow mold machine.

On or about March 28, 2015, a maintenance employee was replacing a hydraulic line and the employer did not ensure a suitable lockout system was used.
