Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer:  (a) 400 Block of East Main Street, Middletown, Pennsylvania - Two employees worked at the bottom of a 6-foot deep trench, dug in Type B soil, to install a fitting on the new water line.  For protection from cave-in in the area of the trench where the employees installed this fitting, the employees used one Efficiency Production single-cylinder rail (hydraulic shoring) with the cylinder installed one foot down from the top of the trench and at least 5 feet up from the bottom of the trench, which did not comply with the manufacturer's tabulated data for installation, exposing the employees to a cave-in hazard, on or about April 9, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
