29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior Rear - Employees were observed walking below a carpenter installing wood flooring on an exterior balcony without wearing hard hats and exposed to being struck by falling wood framing members, on or about 04/09/2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  For the purpose of this inspection, this employer was determined to be the controlling and correcting employer on the job site. The employer is being cited under OSHAs multi-employer workplace policy.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift*:  a) Exterior Rear of Building - An employee observed in the basket of a JLG articulating boom lift basket not wearing a personal fall arrest system attached to the boom or basket was exposed to a 35 ft. fall hazard, on or about 04/09/2015.  For the purpose of this inspection, this employer was determined to be the controlling and correcting employer on the job site. The employer is being cited under OSHAs multi-employer workplace policy.   *Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.
