29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs.      a.  On or about March 27, 2015, located in Mechanical Room 053, employees were cleaning the inside of a 13,200 VAC indoor substation without deenergizing live parts, thereby exposing employees to arc flash/shock hazards.

29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment shall be determined before circuits or equipment are deenergized.    a.  On or about March 27, 2015, located in Mechanical Room 053, employees were cleaning the inside of a 13,200 VAC indoor substation without safe procedures to ensure all circuits had been deenergized on side H of the substation, thereby exposing employees to arc flash/shock hazards.

29 CFR 1910.333(b)(2)(iv)(B): A qualified person shall use test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and shall verify that the circuit elements and equipment parts are deenergized. The test shall also determine if any energized condition exists as a result of inadvertently induced voltage or unrelated voltage backfeed even though specific parts of the circuit have been deenergized and presumed to be safe. If the circuit to be tested is over 600 volts, nominal, the test equipment shall be checked for proper operation immediately after this test.    a.  On or about March 27, 2015, located in Mechanical Room 053, employees were cleaning the inside of a 13,200 VAC indoor substation without verifying that the circuit and equipment had been deenergized to prevent backfeed due to a loop feed system, thereby exposing the employees to arc flash/shock hazards.

29 CFR 1910.137(c)(2)(xii): The employer shall certify that equipment has been tested in accordance with the requirements of paragraphs (c)(2)(iv), (c)(2)(vii)(D), (c)(2)(viii), (c)(2)(ix), and (c)(2)(xi) of this section. The certification shall identify the equipment that passed the test and the date it was tested and shall be made available upon request to the Assistant Secretary for Occupational Safety and Health and to employees or their authorized representatives.                                                                                                                                                               Note to paragraph (c)(2)(xii): Marking equipment with, and entering onto logs, the results of the tests and the dates of testing are two acceptable means of meeting the certification requirement.   a.  On or about March 27, 2015, employees used electrically rated equipment and personal protective equipment while verifying that electrical equipment had been deenergized.  The employer did not certify that electrical equipment had been tested.
