29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about 04/09/2015, at the job site - employees conducting roofing work from a 6:12 sloped roof were exposed to a 14 feet 7 inch and 8 feet 7 inch fall hazard, in that, personal fall protection was not used.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about 04/09/2015, at the job site - the rails of the ladder used to access the roof was extended only 21 inch and did not extend at least 3 feet above the upper landing surface, exposing the employees to a 8 feet 10 inch fall hazard.
