29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: Platform C, Kitchen freezer; on or about 07 April 2015 and times prior thereto; the employer did not ensure that employees working inside a walk-in freezer (make and model unknown) were capable of opening the freezer door from the inside due to a broken latch mechanism. The interior mushroom plunger would not cycle and incapable of opening the door from the inside. The condition exposed employees to sub-zero temperatures capable of causing hypothermia and/or frostbite.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting. Platform C, Slurry room; on or about 07 April 2015 and times prior thereto; an Ingersoll Rand vertical pump was missing a guard on the vertical rotating shaft. An opening measuring nine by nine inches was not guarded. This condition exposed employees to a caught in hazard.
