Switch Gear Room, General Electric Motor Control Center: On or about April 7, 2015 and times prior thereto, the panel marked "SPARE" had a three (3) inch diameter opening at it face. This condition exposed employees to a fire hazard and an electrical shock and burn hazard in the event of contact or ignition from the 480 volt labeled circuit.

This condition exposed employees to minor smoke and fume inhalation hazards.
