29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about April 8, 2015, at N. Labarre Road and 22nd Street, Metairie, LA, the employer failed to ensure that employees wore protective eye wear while using a nail gun and a circular saw. Instance a) An employee was using a nail gun to apply wooden plywood sheeting without wearing protective eyewear. Instance b) The owner of the company was using a Makita circular saw to cut plywood without wearing protective eyewear.

Labarre Road, Metairie, LA, the employer failed to ensure that employees were protected by a fall arrest system while conducting roofing activities. Employees were installing wooden plywood sheeting to a residential structure.
