29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. Platform C, welding shop; on or about 07 April 2015 and times prior thereto; an energized 25 foot electrical cord suspended, from the ceiling did not have a ground. This condition exposed employees to electrical shock and burn hazards.
