29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about March 26, 2015 and times prior thereto: a) Fall protection equipment was not provided and required for employees using the lift in the feed mill at heights greater than 6 feet. The lift is used to access top of bins. b) Fall protection equipment was not provided and required for employees repairing spouts on top of the fertilizer house at heights greater than 6 feet. Employees were exposed to the hazard of falls.
