29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a. Front production room L-1, L-2 Uniloy machines - the employer did not obtain an annual audiogram when the machine operator task was exposed to continuous noise levels at 145% of the allowable 8 hour TWA (85 dBA). The equivalent dBA level of this dose % is 92.6 dBA. The sampling was performed for 290 minutes during one shift. Violation was observed on or about 6/4/15.
