29 CFR 1910.107(c)(6): Electrical wiring, motors, and other equipment outside of but within twenty (20) feet of any spray area, and not separated therefrom by partitions, produced sparks under normal operating conditions and did not otherwise conform to the provisions of subpart S of this part for Class I, Division 2 Hazardous Locations:  a) Spray Painting Area - On or about March 31, 2015, the spray painting area had electrical outlets and electrical appliances that were not rated for Class I, Division 2 locations and were located within five feet of the spray application process.

29 CFR 1910.107(g)(2): All spraying areas were not kept as free from the accumulation of deposits of combustible residues as practical, with cleaning conducted daily if necessary:  a) Paint Mixing Room -  On or about March 31, 2015, the employers have allowed large amounts of paint to accumulate on the walls, floors and equipment used in the paint mixing room. The employer did not routinely clean the deposits of combustible material.   This was observed on March 31, 2015.  b) Spray Booths - On or about March 31, 2015, the employers have allowed the accumulation of paint on the walls of both spray paint booths.  The employer did not clean the waste material which was adhered to the walls of the booth on a routine basis.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) Collision Center - On or about March 25, 2015, the employer provided and required their employees negative pressure, half face, filtering face pieces respirators when mixing paints and spray painting automobiles. The employer did not establish and implement a written respiratory protection program with work-specific procedures.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  Collision Center -  On or about March 31, 2015, the employees were provided and used chemicals such as, but not limited to, 4-chloro-a,a,a-trifluorotoluene and Hexamethylene diiisocyanate. The employer did not provide the employees, upon hire, adequate training regarding the hazards of the chemical products being used in the mixing room and spray painting booths.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  a) Collision Center -  On or about March 31, 2015, the employer did not provide an adequate eye wash station in the production area in the event of an accidental splash to the eyes with such hazardous chemical as 4-chloro-a,a,a-trifluorotoluene and 1,2,4-trimethylbenzene.
