On or about  March 9, 2015, the employer failed to ensure that open sided floors were guarded exposing employees to fall hazards of greater than four feet in the following location:         a)   Interior south side of warehouse open sided floors approximately 72 1/2 inches.         b)   Exterior work platform south side of warehouse adjacent to the crack leg, approximately 96 inches from ground level.        c)    Exterior work platform located above the warehouse. The platforms served as the work platform for employees changing belts on the crack leg elevators.
