29 CFR 1910.145(c)(2)(i): Caution signs were not used only to warn against potential hazards or to caution against unsafe practices. a) Lower Level Entry Door to Orchestra Pit Area - The employer did not use caution signs to warn against potential hazards or to caution against an approximate three and one half foot elevation change between the door level and the bottom of the pit. On April 10, 2015, an employee died of injuries sustained after falling into the pit from the lower level entry door area.

29 CFR 1910.305(j)(1)(ii): Portable hand-lamps supplied through flexible cords were not equipped with a handle of molded composition or other material approved for the purpose, and a substantial guard attached to the lamp-holder or the handle: a) Lower Level Area - The employer provided a portable hand lamp with a metal shell, non-insulated handles, and without a guard for employees to use when illumination was needed.
