29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):      a) A-B Contracting, Inc. @ Newport Landing & Hawkins Road, Fenton, MO 63026: On and before April 10, 2015, creating and correcting employer did not ensure that the protective system was installed along the entire west wall of the excavation. The excavation measured approximately 5 feet in depth along the east wall and approximately 7 feet in depth along the west wall. The upper portion of the west wall, approximately 33 inches, was unprotected. This condition exposed two employees to struck-by hazards.

29 CFR 1926.652(c)(2)(iii): Manufacturer's specifications, recommendations, and limitations, and manufacturer's approval to deviate from the specifications, recommendations, and limitations were not in written form at the jobsite during construction of the protective system and thereafter until the protective system was no longer utilized:    a) A-B Contracting, Inc. @ Newport Landing & Hawkins Road, Fenton, MO 63026: On and before April 10, 2015, creating and correcting employer did not ensure that tabulated data was available for the EPI V-Shoring and 4 feet x 8 feet Sheeting System that was used as a protective system for the excavation. The excavation measured between 5 feet in depth on the east wall and 7 feet in depth on the west wall. This condition exposed employees to struck-by hazards.

The excavation measured 5 feet in depth along the east wall and 7 feet in depth along the west wall. This condition exposed employees to struck-by hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    a) A-B Contracting, Inc. @ Newport Landing & Hawkins Road, Fenton, MO 63026: On and before April 10, 2015, the employer did not provide employees with information provided in Subpart P  Excavations. Employees installed an excavation protective system inside a trench which measured approximately 5 feet deep along the east wall and 7 feet deep along the west wall and were not previously trained to recognize or anticipate hazards associated with working inside an excavation. This condition exposed employees to struck-by hazards.
