On or about March 17, 2015, employees were excavating with pneumatic jackhammers and backfill tampers, without the use of adequate Personal Protective Equipment (metatarsal guards), therefore exposing the employees to foot injuries/amputations from the pneumatic equipment bits. b. On or about March 25, 2015, employees were excavating with pneumatic jackhammers and backfill tampers, without the use of adequate Personal Protective Equipment (metatarsal guards), therefore exposing the employees to foot injuries/amputations from the pneumatic equipment bits. c. On or about March 26, 2015, employees were excavating with pneumatic jackhammers and backfill tampers, without the use of adequate Personal Protective Equipment (metatarsal guards), therefore exposing the employees to foot injuries/amputations from the pneumatic equipment bits. d. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. On or about March 17, 2015, employees were cutting and capping a pressurized natural gas line, during which the employer had not develop and implemented a Lock Out Tag Out (LOTO) Program, nor provided employee training, therefore exposing these employees to potentially hazardous energy, such as but not limited to; thermal burns and explosive/blast type injuries. b. On or about March 25, 2015, employees were cutting and capping a pressurized natural gas line, during which the employer had not develop and implemented a Lock Out Tag Out (LOTO) Program, nor provided employee training, therefore exposing these employees to potentially hazardous energy, such as but not limited to; thermal burns and explosive/blast type injuries. c.
