29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: IN MAIN WAREHOUSE AND YARD: On or about April 13, 2015 and at times prior thereto, employees were exposed to a struck-by hazard while using Mitsubishi Forklift Model FGC45K-LP-C, Serial AF87A10248, with modified forklift tines (nut welded on tine) that significantly reduces the lifting capacity of the tine resulting in possible cataclysmic failure.
