29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Facility: The employer did not implement a lockout/tagout program including annual inspections and training for the employees required to perform maintenance and services on equipment in the plant areas. Employees were or could have been exposed to caught in or struck by hazards.  Employees maintain machines such as, but limited to shapers, moulders, bandsaws, planners, sanders, borers and compressors.       Columbus Cabinet Company, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 478540, citation number 1, item number 3 and was affirmed as a final order on  10/09/2012, with respect to a workplace located at 6047 Coca Cola Blvd., Columbus, Georgia.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Production Area - The employer did not train or evaluate all employees operating the Daewoo and Clark forklift trucks before allowing them to operate the equipment.  Employees were exposed to a struck-by hazard.      Columbus Cabinet Company, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 478540, citation number 1, item number 4 and was affirmed as a final order on  10/09/2012, with respect to a workplace located at 6047 Coca Cola Blvd., Columbus, Georgia.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    a) Paint Booth and Production Floor; Employees were required to work with chemicals such as, but not limited to, paint, lacquerer thinner, alcohol, and propane while manufacturing cabinets.  The employer did not provide training on the new label elements and safety data sheet format.
