29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a) Plant - On or about 04/06/15 and at times prior thereto employees participated in sanitation operations for equipment including (but not limited to) the cuber and the bone mill, and the employer did not provide locks, tags, and/or related hardware as required for lockout and prevention of amputations. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Plant - On or about 04/06/15 and at times prior thereto employees participated in sanitation operations for equipment including (but not limited to) the cuber, the breast conveyor and the bone mill. The steps specified in the written lockout tagout procedures were not followed. Employees relied on other workers for de-energizing and energizing the machinery from remote panels, and verbally communicating the power status. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) Plant - On or about 04/06/15 and at times prior thereto employees conducted sanitation operations on the meat cuber. The chain and sprocket system connected to the electric motor/gear box in the lower part of the equipment was not guarded, exposing workers to amputations. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
