29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    Facility: The employer did not provide formal instruction training to powered industrial vehicle operators that consisted of lectures, discussions, video tapes, written materials, and tests to ensure operators understand the safe operation of the vehicle and the operator's performance in the workplace to prevent struck by hazards.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1200(f)(5): Except as provided in paragraphs (f)(6) and (f)(7) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged, or marked with the identity of the contents contained therein and contains appropriate hazard warning labels:  Facility: Employees were using an unlabeled one liter bottle of welding fluid during the plasma cutting operations and the employer did not ensure each container of hazardous chemical in the workplace was labeled, tagged, or marked with the identity of the chemical.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
