29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Rear of Residence at 4039 White Oak Road, Paradise, Pennsylvania - Employees worked doing siding installation tasks from the roof above the sunroom.  This roof had a 3 in 12 slope, an eave height above the wooden deck of 8 feet 7 inches, and an eave height above the ground of 10 feet 6 inches. The employees did not use any fall protection, exposing them to a hazard of falling to a lower level, on or about April 9, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(7): Ladders that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  (a) Rear of Residence at 4039 White Oak Road, Paradise, Pennsylvania - To access the roof above the sunroom, an employee used a Werner telescoping multi-ladder, in an extension ladder configuration, on a wooden deck that had become wet from rain.  The employee did not secure this ladder, exposing the employee to a fall to a lower level.  While accessing or descending this ladder, the employee fell from the ladder when the ladder slipped out from under him due to the accumulated dampness on the decking, resulting in the employee sustaining a broken left leg requiring hospitalization, on or about April 9, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
