29 CFR 1910.1096(d)(1): The employer did not make such surveys as necessary to evaluate the radiation hazards incident to the production, use, release, disposal or presence of radioactive materials or other sources of radiation under a specific set of conditions: On or about 22 April 2015 employees were exposed to ionizing radiation exposure hazards when the employer failed to conduct surveys of the primary worksite and the clients worksite periodically to ensure that Naturally Occurring Radioactive Materials (NORM) materials were not present at the work place or had been inadvertently allowed into the work place to minimize or eliminate the hazard.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. On or about 22 April 2015 employees were exposed to chemical exposure hazards when the employer failed to ensure that chemicals at the worksite were properly labeled to prevent accidental exposure or unintentional misuse.
