29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) Facility, 865 State Route 210, Shelocta, PA: On or about April 14, 2015 the Case 586G powered industrial truck had defective pins which did not seat properly to hold the forks in place and prevent movement. The truck had not been taken out of service until the pins could be replaced. The employees continued to use the equipment to move sections of pipe and steel plate or other materials as needed.

In addition, several of the cylinders were missing the valve protection cap.
