29 CFR 1910.95(i)(2)(i): The employer did not ensure hearing protectors were worn by employees exposed to sound levels exceeding those listed in Table G-16 of 29 CFR 1910.95(b)(1): On the production floor, employees were exposed to sound pressure levels exceeding 85 decibels and were not wearing hearing protection in the following areas: (a) While mopping the floors between presses #111/#110, an employee was exposed to continuous noise levels of 93.3 decibels and was not wearing hearing protection. (b) While operating the 1000-ton press #110, an operator was exposed to continuous noise levels of 95 decibels and was not wearing hearing protection.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: In the production area, along the north and west sides of the building, exit lighting signs above the emergency exit doors along the east wall were not illuminated or working properly. D&D Manufacturing Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.37(a)(4), which was contained in OSHA inspection number 979337, citation number 02, item number 001 and was affirmed as a final order on December 23, 2014, with respect to a workplace located at 1330 Pullman Dr. El Paso TX 79936.
