OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and/or strike-by hazards while handling a load with an overhead crane: Decontamination Room: The employer did not ensure through visual observation, specific instructions and training that the load/a motor, which was hoisted up a portion with an overhead crane, was positively and effectively secured and balanced by appropriate means to prevent from slipping and/or falling/dropping. On or about April 10, 2015, the partially suspended load, " H.B. Robinson vertical motor inner race" was dislodged from the rotor and fell on an employee's right hand, resulting in a finger amputation.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and/or strike-by falling loads from an under hung overhead crane: Decontamination Room: The 15-ton, Appli No 00021 under hung hoist overhead crane had the following deficiencies: 1. The pendant control was not completely marked. ABATED DURING THE INSPECTION. 2. The latch on the hook was bent. 3. The top sheaves were bent.
