OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from falling loads. a. On or about 04/14/15, in the Pressure Testing Area at the workplace, employees were exposed to crushing hazards from radiators secured to the load block by devices that were not rated for lifting or hoisting. Employees used an in-house fabricated device to lift radiators that exceeded 500 lbs.

29 CFR 1910.101(b): Compressed gases. The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6 Section 3.4.5., Compressed Gas Association Pamphlet P-1-1965, as adopted by 4 OSH 1910.101(b) Suitable Pressure Regulating Devices must be used in all cases where gas is admitted to the systems having pressure rating limitations lower than the cylinder pressure a. On or about 04/14/15, at the workplace in the Testing Area, a suitable pressure regulating device was not used to regulate compressed nitrogen gas that was used while pressure testing radiators. The regulator's scale of adjustment was 0 to 3000 psi when 400 psi was the maximum pressure used to test radiators.
